Chronic Conditions Prevention and Integrated Care Grants Program

Funding is available under two tiers:

  • Tier 1 – National, Collaborative Activities
    • $1.25 million to $3 million per year
    • Up to $12 million total over ~3.5 years
  • Tier 2 – Targeted Activities
    • $375,000 to $1.25 million per year
    • Up to $2.5 million total over ~1.5 years

Funding amounts depend on project scope, complexity and tier.


The Chronic Conditions Prevention and Integrated Care Grants Program funds initiatives that improve health outcomes and quality of life for Australians living with, or at risk of, chronic conditions. The program prioritises early intervention, prevention, and integrated care models that deliver scalable and sustainable health improvements.

Key Requirements

  • Projects must align with the National Strategic Framework for Chronic Conditions 2026–2035.
  • Tier 1 projects must demonstrate national delivery across all states and territories.
  • Activities must address at least:
    • Two objectives (Tier 1)
    • One objective (Tier 2)
  • Projects must not duplicate existing publicly funded programs.
  • Applicants must comply with:
    • Commonwealth Child Safe Framework
    • Workplace Gender Equality Act 2012
    • Multicultural Access and Equity Policy

What is eligible for funding?

Eligible activities include:

  • Health promotion and education initiatives
  • Prevention and early detection programs
  • Integrated, multidisciplinary care models
  • Multimorbidity management approaches
  • Targeted support for priority populations

Eligible project activities may include:

  • Systemic reform aligned to program objectives
  • Policy linkages, partnerships and co‑design
  • Clinical or consumer education and awareness
  • Service delivery to address care gaps
  • Community engagement and consultation
  • Workforce development and training
  • Data collection and evaluation activities

What companies are eligible for funding?

Eligible applicants include:

  • Aboriginal and Torres Strait Islander Corporations
  • Companies
  • Cooperatives
  • Incorporated Associations
  • Statutory entities
  • Partnerships

Additional eligible organisations:

  • Non‑Government Organisations
  • Primary Health Networks
  • Community organisations
  • Academic institutions

Is this grant competitive or entitlement based?

Competitive. Your application will be assessed among other applications by a judging panel.

How can I increase my chances of winning this grant?

  • CAPEX (Capital Expenditure) cost-benefit analysis
  • Stakeholder relations and Risk Mitigation Plans
  • Any other relevant forecasting that will support your application
